Below, you will find important forms to complete prior to our meeting.
We would ask that you have a few things with you at the start of our meeting including:
A glass of water
Tissue
Coloured pens or pencils
Printed copies of the worksheets you will find below
A sticky note or an elastic band that fits on your wrist (something that you can use as a reminder).
Please print the documents below prior to the group and have them easily accessible when we begin. You will need printed copies of all three of the worksheet pages to participate in the STEP process. If you do not have access to a printer, please draw the pages before our meeting.
After completing the STEP Together process, we will ask you to complete this form online.
